Get your Medical Certificate in three simple steps!

  • Fill out a 1-page brief health history

  • Complete your DOT physical exam with Dr. Christopher Venegas, DC, QME a certified medical examiner for FMCSA.

    • Medical History Review

    • Review height, weight, blood pressure, pulse rate, range of motion, breathing, eyes, ears

    • Discuss your current medical conditions

    • A Basic Urinalysis test

  • We complete your paperwork. Photo ID required

    • You get a DOT Medical card for your Commercial Drivers License(CDL) to keep with you at all times.

    • You get a copy of the DOT physical form for your records.

    • We will keep a scanned copy of your DOT physical form in our office.

 THINGS TO PLAN IN ADVANCE:

If you have a pre-existing medical condition you’re going to need some to bring some additional paperwork and information:

  • Diabetes – Bring a list of current medications and the name of prescribing Doctor.

  • Elevated Blood Pressure - Bring a list of current medications and name of prescribing Doctor.

  • Heart Condition – If you’ve had a heart attack or cardiovascular disease, you’ll need to bring a copy of your most recent exercise stress test.

  • Sleep Apnea – Bring a copy of your latest sleep test results.

  • Glasses, contacts, and hearing aid if your currently use any

  • Medications – A current list of all the prescription medicines you take, including strength and dosage. If you regularly take over-the counter medications, such as antacids or allergy pills, list them, too.

  • Contact information – Have names and phone numbers of your doctors in case you need to call and have them fax missing information.

THE DAY OF THE DOT/CDL PHYSICAL EXAM:

  • Remember to bring the medical records you’ve prepared in advance, including your list of medications.

  • Remember to bring; eye glasses and /or hearing aid, and your Drivers License.

  • Drink water. Come with a need to pee.


Dr. Christopher Venegas, DC, QME is a registered medical examiner on the Federal Motor Carrier Safety Administration’s National Registry of Certified Medical Examiners. He has completed the required training and testing concerning the Federal physical qualifications and standards for truck and bus drivers and possesses the necessary knowledge and professional credentials to perform physical qualification examinations for commercial motor vehicle drivers in accordance with the Federal Motor Carrier Safety Regulations (49 CFR 391.41 – 391.49).
